The initiative, which successfully piloted at Bedford Hospital earlier this year, identifies patients who require new care or increased carer support upon leaving hospital. Luton Borough Council will also launch its own 'Home First Recover to Reable' scheme.
Under the scheme, a home care provider will deliver a universal offer of four care calls per day for the first 72 hours, or up to one week post-discharge depending on the local authority. This aims to facilitate a safe return home, with needs assessed in the patient's own environment to arrange further care.

Feedback from patients during the pilot indicated a strong preference for recovering at home, with many expressing relief and improved wellbeing. One patient discharged via the scheme stated, “I would definitely recommend this to others in hospital as it allowed me to go home and have confidence.”
